Volunteer Agreement Form

I and/or my minor child (junior volunteer), the Volunteer, desire to work as a volunteer for IntelliChoice Organization and engage in the activities related to providing IntelliChoice On-site Tutoring Program to underprivileged children.

By signing this liability waiver and release, I and/or the parent or legal guardian of a junior volunteer agree(s) to the following:
1. Release and Waiver
My and/or my minor child’s participation in the IntelliChoice On-site Tutoring Program is voluntary. I understand that the activities related to this program may involve accidental injury and hereby voluntarily assume such risks. Knowing these risks, I want myself and/or my minor child to participate in this program. I hereby release, waive, discharge and covenant on behalf of myself and/or my child not to sue the IntelliChoice Organization, its officers, Branch Managers, employees, representatives, volunteers, affiliate agents or sponsors (hereinafter collectively “Released Parties”) from any and all liability to me, or my minor child, for any accident, personal injury, property damage, any other circumstance or incident occurring during tutoring session or as a result of my, and/or my minor child’s participation, or actions of any kind which may hereafter occur, including traveling to and from this activity, whether caused by Released Parties’ negligence or otherwise.
2. Indemnification and Hold Harmless
I further agree to defend, indemnify, and hold the Released Parties harmless from and against any and all losses, liabilities, damages, costs or expenses that may arise by reason of services I, or my minor child, provide as a volunteer or that are connected in any way herewith. I will pay all cost incidents to any claim including, without limitation, attorneys’ fees.
I expressly agree that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the law of the State of Texas and that if any portion is held invalid, it is agreed that the balance shall continue to have full legal force and effect.
3. Photographic Release
I understand that while participating in the IntelliChoice On-site Tutoring Program, I may be photographed. I agree to allow my photo, video, or film to be used for any legitimate purpose including promoting services and programs by the activity holders, producers, sponsors, organizers, and assigns.
4. Other
I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness to myself and/or my minor child during this activity.
I have read and voluntarily sign this electronic release, waiver of liability and indemnity agreement and further agree that no oral representations, statement or inducement apart from the foregoing written agreement have been made. Typing the name listed below shall have the same force and effect as my signature and by typing such name below and submitting this application form, I agree to all terms and conditions stated herein. If you do not type your name below then your application will be considered incomplete and will not be processed.
